Toilet cleaner near to-go cups, expired chicken salad found at Phoenix-area eateries

PHOENIX (AZFamily) — Every week, Arizona’s Family looks up restaurants recently inspected by Maricopa County’s Environmental Services Department. Restaurants with some of the highest numbers of “risk factors,” considered major health code violations by inspectors, are selected for our Dirty Dining Report. Here are the Valley restaurants that did not make the grade for April 30, 2025:

Poké Wave - 9920 S Rural Rd, Tempe

3 violations:

• No certified food manager
• Employee with gloves touched clothing, then picked up something off floor without washing hands
• Raw fish roe stored above cucumbers

Against the Grain Food - 124 W Main Street, Mesa

3 violations:

• Butter not kept cold enough
• Chicken salad kept past discard date
• Organic debris on hood above stove

Kay Sushi - 10115 E Bell Rd, Scottsdale

3 violations:

• Workers not washing hands properly
• Raw shrimp stored over bottles of sauces
• Dishwasher not cleaning properly

Black Rock Coffee Bar - 1694 E Riggs Rd, Gilbert

3 violations:

• Employee touched hair then prepared drink without washing hands
• Whipped cream cheese at room temperature
• Bottle of toilet cleaner stored above to-go cups

Smashburger - 6807 E Baseline Rd, Mesa

3 violations:

• Employee beverage stored on prep table
• Melted butter not kept hot enough
• Cut lettuce not cold enough
