Dishes not cleaned properly and excess food debris on cutting boards are among the violations at Phoenix area restaurants.

PHOENIX (AZFamily)Every week, Arizona’s Family looks up restaurants recently inspected by Maricopa County’s Environmental Services Department. Restaurants with some of the highest numbers of “risk factors,” considered major health code violations by inspectors, are selected for our Dirty Dining Report. Here are the Valley restaurants that did not make the grade for Dec. 18, 2024:

Bosa Donuts - 13820 W Glendale Ave, Glendale

3 violations

  • No log for when ham and cheese croissants were made
  • Boba tapioca balls not stored correctly
  • Scented bleach used to clean dishware

Well Seasoned - 4280 N. Drinkwater Blvd, Scottsdale

3 violations

  • Raw eggs stored above ready-to-eat foods
  • Dishes not sanitized correctly
  • Cooked oxtail out of temperature

Zu & Pocha - 1212 E Apache Blvd #107-109, Tempe

3 violations

  • Raw fish stored over cooled eel
  • Cheese and cabbage kept past discard dates
  • Hot water not hot enough

Chutneys Indian Express - 925 E Apache Blvd, Tempe

4 violations

  • Employee put gum in mouth then handled clean dishes
  • Bowls not cleaned correctly
  • Curry and butter not kept hot enough
  • Pesticides stored near clean dishes

Ono Hawaiian BBQ - 4972 S Power Rd, Mesa

8 major violations

  • No certified food manager
  • Staff member not washing hands properly
  • Cook handling chicken then not washing up
  • Hand sink blocked by trash can
  • Coconut stored under raw chicken and beef
  • Excess food debris on cutting boards
  • Short rib not held at proper temperature
  • Slaw mix and spam not kept cold enough
