PHOENIX (AZFamily) — Every week, Arizona’s Family looks up restaurants recently inspected by Maricopa County’s Environmental Services Department. Restaurants with some of the highest numbers of “risk factors,” considered major health code violations by inspectors, are selected for our Dirty Dining Report. Here are the Valley restaurants that did not make the grade for Oct. 16, 2024:

- Cooked pumpkin not cooling properly
- Cooked rice not kept hot enough
- Cheese and milk out of temperature
Biscuits Café - 310 E Bell Rd, Phoenix
3 violations
- Tongs placed in hand sink next to waffle station
- Bags of biscuits not stored in proper containers
- Cheese and butter out of temperature
Subway - 8345 W Thunderbird Rd, Peoria
4 violations
- Inner glass of microwave oven shattered, exposing shards of glass
- Plumbing not installed properly
- Mop sink faucet broken and leaking
- Women’s restroom door lock broken
The Original Empanada Shop - 21824 S Ellsworth Road, Queen Creek
7 violations
- Cook rinsing dishes then preparing food
- No paper towels at hand wash sink
- Raw eggs stored above red sauce
- Cook not cleaning pans properly
- Containers of beans not cooling correctly
- Salsa and banana pudding kept past discard dates
- Chorizo and sour cream not stored at proper temperature
